Deliver whole-person care in 30- to 60-minute appointments, supported by a health coach and care team, focusing on high engagement, managed care, and culture transformation to reduce costly service utilization and improve overall health outcomes.

Position Highlights: See 10-15 patients daily in longer, meaningful appointments. Collaborative, team-based approach with health coaching support. Focus on proactive, value-based care

Compensation & Benefits:
  • Salary: $238,000 - $350,000 + bonus eligibility

Requirements:
  • MD or DO degree with completed Family Medicine or Internal Medicine residency (Family Medicine preferred)
  • Board Certified by ABMS (ABFM/ABIM) or AOA
  • At least 3 years of primary care experience
  • Current, valid medical license and DEA number
  • Proficiency with EMR systems (NextGen preferred), coding, and Medicare Advantage/HCC documentation
  • Experience with Google Workspace and team-based care
